What does your insurance pay?
Whether the costs of mobile blood sampling are reimbursed depends on your insurance status. Here you will find the routes for patients with statutory or private health insurance and for self-payers, with suggested wording for your cover letter.
How to get the costs covered
Blood sampling at home with subsequent transport of the samples to the laboratory is not yet part of the standard benefits catalogue of the German statutory health insurers as a combined service.
1. Individual application for benefits in kind (Einzelfall-Sachleistungsantrag)
Therefore, apply to your health insurer for cost coverage before you order from Creasano. This application is called an “Einzelfall-Sachleistungsantrag” (individual application for benefits in kind).
Send it via your health insurer’s app, by email or by post (copies). Enclose these two medical documents:
A laboratory request (Muster 10) for the requested laboratory parameters
A private prescription for “mobile blood sampling at home including laboratory courier service” with the individual indication, the reason why you cannot come to the practice, and a note that the practice cannot provide the home visit in time (see sample prescription)
The prescription does not name a provider. As part of the application, your health insurer has the prior right to name a provider to carry out the service.
2. Right to self-procurement under Section 13(3)/(3a) SGB V
If the health insurer does not decide in time or wrongly refuses a necessary service, Section 13(3)/(3a) SGB V governs when costs are reimbursed. There is no automatic entitlement.
3. Individual application for reimbursement (Einzelfall-Kostenerstattungsantrag)
Your health insurer did not name a suitable provider in time, so you commissioned CREASANO yourself. The blood sample was taken and you have paid the invoice.
Now apply to your health insurer for reimbursement of the costs. This application is called an “Einzelfall-Kostenerstattungsantrag” (individual application for reimbursement).
Send the application via your health insurer’s app, by email or by post (copies). Enclose the following documents:
Invoice(s)
Proof of payment, for example a bank transfer confirmation
Your health insurer decides whether to reimburse the costs after reviewing your application.
Expect to cover the costs privately at first. Whether and to what extent your health insurer covers the costs is decided case by case.

Pricing is based on Section 6(1) GOÄ by analogy
The combined service is not listed as a single item in the GOÄ. Billing is therefore based on comparable medical services.
Requirement for placing an order: a laboratory request and a private prescription from your treating doctor. Alternatively, the laboratory parameters can be listed directly on the private prescription.
You receive two private invoices, pay them yourself as usual and submit them to your private health insurer or your civil service allowance office (Beihilfe).

Initially, expect to cover the costs privately
Reimbursement is not guaranteed. It depends on your individual insurance and your plan.
Privately commissioned care
You urgently need a timely blood sample and do not intend to submit the invoices to your statutory (GKV) or private (PKV) health insurer for reimbursement? You bear the costs yourself. That is no problem.
However, our mobile blood sampling may only be carried out on the basis of a medical laboratory request and a private prescription from your GP, specialist or alternative practitioner (Heilpraktiker). CREASANO UG does not arrange blood sampling without medical necessity and a prescription.
You could also use a “direct laboratory”
Many patients do not know that there are so-called direct laboratories. There, you can book an appointment without a prescription from your own GP or specialist and have your blood taken on site and receive the laboratory results.
One well-known direct laboratory is Labor Staber, for example, but many other laboratories now offer this service too. Simply check their websites or give them a call.
